I understand that participation in this study involves registering an email address to access the Quitex website and completing the ‘Five Step Plan’. This will take approximately 20 minutes. I understand that when I complete the ‘Five Step Plan’ I will be sent an email with my personal ‘Quit Smoking Planner’ and will be required to complete a short questionnaire about my smoking, which will take approximately 10 minutes. I understand that I will be contacted by my email again in four weeks to complete another questionnaire about my smoking, which should take approximately 10 minutes. I also understand that at this point I may be offered the opportunity to provide a saliva sample, for which I will need to provide my address.

I understand that I must be 18 years of age or older to take part in this study.

I understand that participation in this study is entirely voluntary and that I can withdraw from the study at any time without giving a reason.

I understand that I am free to ask any questions at any time. If for any reason I experience discomfort during participation in this study, I am free to withdraw or discuss my concerns with the Quitex research team (quitex@cardiff.ac.uk).

I understand that the information provided by me will be held confidentially, and that only the Quitex research team can trace this information back to me individually. The information will be obtained and processed for the specified reasons only, and will not be kept for longer than one year beyond data analysis. Following this the information will be deleted. I understand that I can ask for the information I provided to be deleted at any time and, in accordance with the Data Protection Act, I can have access to the information at any time.

I also understand that at the end of the study I will be provided with additional information and feedback.
